Role & Responsibilities

Provide technical support to ensure implementation of quality medical services within the health interventions in Renk, with a focus on diagnosis and management, in line with South Sudan and internationally relevant treatment guidelines. The Medical Manager will primarily act as a focal point for quality improvement in health care delivery in Renk.

Project Overview

Renk County is located in Upper Nile State, and is situated on the border with Sudan. Despite general stability in the area, the local health system remains fragile and there are ongoing population movements. There is a high prevalence of communicable diseases like malaria and acute respiratory infections, and high levels of malnutrition.

Medair’s aim in Renk County is to reduce morbidity and mortality in vulnerable displaced and host populations through the provision of emergency WASH (water, sanitation and hygiene), healthcare, and nutrition services. Medair operates two primary health care clinics in Abayok and Wunthou, which include outpatient, nutrition services, and reproductive health care. Essential maternal and newborn services, towards implementation of the Minimal Initial Service Package, are provided, including antenatal and postnatal care. In addition, in Abayok clinic Medair provides 24-hour skilled delivery services and an inpatient Stabilisation Centre for severely malnourished children with complications. Medair has been providing these services in Renk County since 2011 and remains one of the only health actors, and the only long-term nutrition partner.

Medair is also implementing integrated Community Case Management (iCCM) in remote villages where access to healthcare is difficult, and has set up Care Groups through which behavioural change communication messages are cascaded to the community.

Key Activity Areas

Quality Management

  • Support improvement of the clinical, technical aspects of health care delivery in Medair’s Health programmes in Renk, in line with South Sudan and relevant international guidelines. This includes Abayok Clinic and Wunthou Clinic (Outpatient Department, EPI and Reproductive Health). Areas of focus include child health and integrated management of childhood illnesses, reproductive health and communicable diseases, including COVID-19 preparedness and response.
  • Monitor the quality of services at health facilities and identify gaps, problems and solutions, in collaboration with the Health Project Manager, health team and facility staff.
  • Provide technical support to clinicians and other health staff, through on the job training, one-on -one clinical supervision and group training sessions.
  • Engage directly with health staff at the clinics, review clinical guidelines and protocols and discuss patient treatment plans where needed.
  • Conduct regular supervision assessments on individual clinicians.
  • Develop and update resources to support clinicians and health staff, including job aids.
  • Work closely with the Health Project Manager and health team to monitor medication prescription and promote rational use of medications.
  • Provide feedback on and input into Medair’s quality improvement activities and reporting tools.
  • Work across other Medair sectors in Renk, including nutrition and WASH programming to support clinical improvement activities that impact health outcomes.
  • Perform other tasks as required, as requested by the Health Project Manager.

Communication

  • Develop and maintain appropriate communication with the team, especially Health Project Manager, Health Managers and Clinic-In-Charge.
  • Participate and proactively engage in health team and facility meetings.
  • Represent, where necessary, Medair at relevant local level coordination meetings involving local authorities, UN agencies and other NGOs.

Key Activity Areas

Nutrition Project Management

  • Manage the assigned nutrition project(s) in order to meet the project objectives within budget and within the allotted time frame and report promptly any operational concerns to line manager and country nutrition advisor (e.g. projected failure to meet objectives; increased beneficiary needs; projected spending on project budget).
  • Provide strong leadership of the assigned nutrition project(s), working to ensure short, mid and long-term positive impacts and outcomes for the beneficiaries, local nutrition authorities (where applicable) and local nutrition staff.
  • Contribute to development and implementation of an appropriate and effective nutrition strategy including proactive, responsible exit planning in consultation with other nutrition staff and field managers.
  • Set clear objectives and indicators for nutrition activities in collaboration with the country nutrition advisor, nutrition staff, field managers and, where appropriate, with the local communities.
  • Continuously monitor and supervise nutrition activities, evaluating progress through outputs and impacts using both quantitative and qualitative data, and involving the nutrition team in analysis and taking appropriate action.

  • Facilitate regular meetings to assist information sharing between project staff, to ensure all staff members are aware of project objectives and current work plans and to provide the opportunity for feedback.
  • Provide coaching and technical supervision to staff to develop ownership & full responsibility for nutrition activities.
  • Assess the training needs of nutrition staff and ensure that appropriate training is conducted.

Financial Management

  • Manage the nutrition budget for the assigned project(s) and ensure all expenses are according to budget and meet the Medair standards with regard to financial management, including accountability and good governance.

Communication and Coordination

  • Develop and maintain appropriate, regular, transparent and supportive communication structures with the assigned nutrition team, Medair in-country and GSO health and nutrition managers and advisors and other relevant stakeholders (e.g. beneficiaries, community leaders, local and national government officials, UN agencies and other NGOs), with the objective of ensuring good cooperation and partnerships.
  • Participate in nutrition coordination meetings, Medair internal trainings and other working groups or meetings as required, representing Medair and feeding back to the nutrition staff and field managers on relevant issues.

Logistics

  • Support the logistics activities of nutrition staff, particularly in relation to the purchasing of medicines, supplies and equipment for the assigned nutrition facilities and activities.
  • Liaise, as necessary, with the nutrition, health, logistics and finance staff to ensure items are procured, transported, stored, managed and accounted for correctly.
  • Submit timely requests to nutrition partners (UNICEF, WFP) to release core pipeline nutrition supplies and monitor supply provision. Ensure nutrition buffer stocks procured and available to prevent programme stock outages.
  • Ensure that all the programming supplies are procured in time for the smooth running of the nutrition activities.

Quality Management

  • Promote and use the Medair intranet, Medair people and other operating procedures, ensuring that all standardised formats are used and guidelines are followed.
  • Ensure nutrition projects are implemented in line with donor proposals and requirements and in accordance with Medair, donor, country and international standards i.e. Sphere and HAP standards.

Key Activity Areas

Health Management

  • Oversee the implementation and management of the Reproductive Health units in line with the activity plan and time frame and any applicable guidelines and standards. Conduct daily field trips to the clinics.
  • Ensure that regular monitoring and evaluation assessments are conducted both against project objectives and to ensure the quality of the programme, with reports being made to the relevant field managers.
  • Oversee the process of data collection, collation and dissemination to meet the requirements and deadlines.

Currently, International Medical Corps supports 2 Hospital and 35 primary health care facilities. The organization is composed of over 750 national staff and 49 expatriates. In addition to comprehensive primary and secondary health care, International Medical Corps also implements multiple complementary programs including mental health, reproductive and sexual health, GBV prevention and response, and health outreach activities. International Medical Corps also supports three midwifery and nursing colleges in South Sudan in Juba, Kajo Keji and Wau. Further, International Medical Corps has a wide-reaching comprehensive health program that covers Juba PoC, Malakal, Maban, Wau and Nyal.

One major area of concern in the current national response is facility management of severe and critical COVID-19 cases. Accordingly, the Ministry of Health has developed a Concept Note requesting for the WHO led International Emergency Medical Team to support South Sudan address some of the challenges.

Job Purpose

The Anesthesiologist will be part of a specialized team to support the Ministry of Health in South Sudan in setting up Intensive Care Units for the management of critical patients requiring respiratory support. As part of the team, he/she, (hereafter referred to as ‘he’) will conduct initial assessment of the designated health facilities, outline needs and resources and carry out necessary on job training for the staff working in these facilities.

Main Responsibilities

To perform this job successfully, an individual must be able to perform each essential function with or without reasonable accommodation. The tasks listed are representative of the nature and level of work assigned and are not necessarily all-inclusive.

  • Carry out a need assessment in the health facilities to determine the needs for equipment, medications and resources required for setting up an Intensive care facility.
  • Ensure that all necessary drugs and material are available for intubation, resuscitation and critical care management of patients admitted in the unit.
  • Draft a training needs for the health facilities and develop a training plan in accordance with available resources. Provide on job training on intubation, ventilation and Basic and Advance Life support to the staff working in the health facilities.
  • In collaboration with the ICU physician, organize the tasks of personnel in the ICU, and draft protocols and Standard Operating Procedures for patient management in the identified health facilities.
  • In collaboration with the Biomedical Engineer, ensure good functioning and maintenance of OT equipment (oxygen extractor, ventilation, equipment, etc).
  • Ensure transfer of patients to the recovery room, and full recovery of the patient’s consciousness and setting analgesic protocol
  • Advise on Postoperative care in both Intensive Care Unit and wards with the responsible medical staff, including fluid management, oxygenation monitoring and airway care and chest physiotherapy and mobilization, with special focus on patient’s pain relief during the recovering process.
  • Implement and supervise hygiene rules, procedures and protocols in OT/recovery, anesthesia material (decontamination, sterilization, storage conditions, etc.) to ensure the safety and protection of patients and staff.
  • In collaboration with the team leader, ICU physician and the hospital Director, develop standard list of medications, items and equipment required for effective functioning of the ICU.
  • Develop SOPs for use of narcotics and anesthetic drugs and train the staff on patient safety, and patient centered approach.

Key Objectives:

Provide support to the EVD preparedness and response – projects in Western Equatoria state

Build capacity of health workers in EVD cases management

Supervise and mentor the Ebola Preparedness and Response Coordinators to discharge their duties as per the EVD preparedness and response plan.

Ensure that there is high standard EVD preparedness related operations in EVD Isolation units, EVD hold units, health facilities and BH/POE sites

Liaise with the State Ministry of Health and WHO to coordinate World Vision’s preparedness and response in line with National Guidelines and in collaboration with other agencies, including the health cluster

Manage the development and revision of EVD program development and implementation in WEQ in line with the WVSS EVD preparedness and response plan and South Sudan Ebola Virus Disease (EVD) Preparedness Plan.

Support the EVD coordinator in ensuring that the Screening Points (SP) are smoothly run and provide technical support for health workers including provision of capacity building on SOP for EVD screening

Liaison with Juba based team and provide regular update on the ongoing EVD preparedness and response program in WEQ

Liaise the WVSS Supply Chain, WHO, UNFPA, UNHCR, UNICEF and the SMOH in ensuring that the EVD Team in WEQ have all the supplies and logistical support for a well-coordinated EVD preparedness and response.

Support the Ebola Preparedness and Response Coordinator to set a strong referral system for suspected cases

Ensure that the EVD preparedness and response is well mainstreamed in the ongoing HPF activities, particularly MNCH activities.

Work with the HPF community mobilization officer and Primary health care officer in ensuring that EVD awareness activities are carried out efficiently and ensure that MTMSGs, Men Champion groups, BHCs, the local community leaders and faith leaders are involved in the EVD sensitization/ awareness.

Ensure infection prevention and control in health facilities and Maternal and Child Health is coordinated with EVD IPC

Qualification: Education/Knowledge/technical skills & experience required:**

The following may be acquired through a combination of formal or self-education, prior experience or on-the-job training:

  • Clinical background (Nurses or Medical doctor), Post graduate studies in public health or epidemiology.
  • Practical experience in Ebola situation (having worked in Ebola outbreak situation) in West Africa or Democratic Republic of Congo.
  • An experience technical EVD Case Management person.
  • Vast experience in budget management and project reporting.
